FAQ

Which health insurance do you accept?
We are a referral based practice. In conjunction with your referral, our staff will work with you, your insurance company and your provider to verify eligibility and benefits.

Does your billing office work with private patients?
Your health is extremely important and we are happy to develop a payment plan for you and your family. Please contact our billing office at 1-877-708-1119 and our staff will gladly assist you.

Do you accept children as new patients?
Referrals for patients under the age of 18 will be reviewed on a case by case basis.

What should I bring to my appointment?
Please bring your driver license and insurance card(s). In order for us to provide your best care, it is incredibly important that we know all of your current medications (over the counter and prescription) as well as supplements including dosage and frequency of use.

How early should I arrive for my appointment?
Please arrive a minimum of 15 minutes early for your appointment.

How much notice is needed to cancel my appointment?
Procedure: you may be charged for a late cancellation. Office visit: In new patient forms you will sign a no show agreement outlining the notice requested. Our office is extremely busy and we appreciate you cancelling in time for another patient to be scheduled. We will work with you to get you in as soon as possible, however, it may be several weeks before the next available appointment.
